The service dog program is a high-quality, high-intensity training program intended to turn appropriate candidates into certified service dogs. This process can take from 6 months to 2 years and can cost thousands of dollars. There is also a high rate of dismissal; meaning, there are no guarantees that your dog will graduate the program once we start. There is a lot of preparation that needs to take place including consulting with your doctors and therapists before you start this process. Please take the time and consider going down this path of service dog training is right for you and your situation before you apply.

Be aware that there is a $50 application fee to submit this form. Applicant dogs must have obtained their AKC CGC title or have completed obedience training through Jay’s Friends Dog Training, or another accredited trainer. Verification may be required.
